If you would like to give us permission to publish your photograph, video, or voice on our own web site, ebook, YouTube video, dvd, or app.
Please note unless otherwise stated there is no payment for getting photographed.
This form is for you being in a photograph if you are 13 or older  – see parent form to give permission for your child under 18. It is for photographs taken by us or on our behalf which  Nathan Steffenson or Nate’s Computer Services is the copyright holder.
